My kids love cake and ice cream of all flavors, and would be very happy with any of those choices.
I picked 'other'; we'd probably do yellow cake with chocolate frosting and vanilla ice cream, but again, I think at the end of the day as long as the birthday girl/boy is happy, it's a success!
DS doesn't like chocolate cake. So he would choose white cake with vanilla ice cream.
Didn't vote cause my kids aren't in that age range. DS would be thrilled w any flavor cake. DH and I too. If people don't like the flavor of cake then.......too bad for them!
I let my kids pick for their birthday. None like chocolate cake or chocolate ice cream. DD would just eat cake.

DS1--food allergic would bring his own treat, DS2--chocolate everything, DD--allergic to chocolate, enjoys vanilla.
Personally whichever cake flavor you choose, I would serve both chocolate and vanilla ice creams so if you have some gluten sensitive kids or kids who are picky about cake, they could enjoy some ice cream.

I am a major chocolate fan, so I would be thrilled with chocolate cake and chocolate ice cream, but I know I am in the minority. I voted to have both flavors of ice cream. I like the idea of a marble cake, is that an option at Costco?
ETA: DD's preference changes with the wind. Some days she prefers chocolate and other times it's vanillia lol.

Our local grocery where everyone gets sheet cakes for birthdays (and weddings) does a half chocolate half yellow. The icing is all of ine - so you wouldn't know that there are two different favors and the cake is split half and half (except for the penny size of chocolate icing on one corner showing). My dd will politely decline yellow cake at a party- she onky likes chocolate. But the split cake is so popular.
At that age my kids loved marble cake with white frosting. And the majority of their little friends always seem to pick plain-old vanilla ice cream. It always seems with my kids and their friends, the simpler the flavors, the better. I always seem to have lots of chocolate cake chunks leftover with the frosting liked off so I just don't do chocolate any more. hth
