I think, if I'm remembering correctly, player one logs in and then player 2 has to hit START on their controller to add to the game.
We have an Xbox One so it may vary, but my kids start a game and then the second kid hits the menu button (three lines) on the second controller to enter the game. I did have a friend who discovered that you can NOT play multiplayer without an HDMI connection or maybe it was an HD capable tv. Her older tv had to be upgraded for the kids to play multiplayer MineCraft. We both were skeptical at first, but it turned out the GameStop employee who told her that was correct.
Yes you have to have an HD TV to play multiple players
