Online methods to collect data from group members?

Our Cub Scout Pack is hosting an event this summer that requires us to collect emergency contact info in advance. We'd like to do this online if we can. I don't think sign-up genius will accommodate this and Survey Monkey isn't probably the best either (but I believe it will work). Any other methods/apps/sites you've used to send out one link and have people provide open ended info?

Thanks! I've never tried Google Forms before but it looks easy. I'll have to test out the output - you just have to access your google account to see it?

Love Google Forms! Yes, just look for it under your Google drive.
