DD1 going into middle school /6th grade. She is supposed to read 4 books of her choice this summer. It hasn't been an issue as she loves to read. Dd1 is going into the 2nd grade. Our elementary school doesn't have any summer requirements.

Yes to the required reading list - at our school (K-12) there is required reading for kids entering grades 4 and up. There is also required work for other classes (e.g. World Cultures in grade 8 and AP classes in HS).

    DS1, entering grade 8:
Reading: The Outsiders (S.E. Hinton) and Quicksilver (Stephanie Spinner)
There will be a test on Quicksilver the first week of school (handwritten notes are encouraged); and there's an essay for The Outsiders
World Cultures: watch 2 out of 5 movies from this list (Spartacus, Ben-Hur, Cleopatra, The Greatest Story Ever Told, The Ten Commandments)
They need to be prepared to discuss the movies.

    DS2, entering grade 6:
Reading: Must read Blizzard!: The Storm that Changed America (Jim Murphy) and choose 2 from a list
He chose Red Pyramid (Rick Riordan) and Walk Two Moons (Sharon Creech).
    List:

    Hatchetby Gary Paulsen
    Magician’s Elephant by Kate DiCamillo
Bridge to Terabithia by Katherine Paterson
Walk Two Moons by Sharon Creech
    Ruby Holler by Sharon Creech
Roll of Thunder, Hear My Cry by Mildred Taylor
Al Capone Does My Shirts by Gennifer Choldenko
Boy at War by Harry Mazer
    Runaway Twinby Peg Kehret
Summer Ball, Heat, Game Changer, & Travel Team ALL by Mike Lupica
The Lightning Thief by Rick Riordan
    Red Pyramid by Rick Riordan
Elijah of Buxtonby Christopher Paul Curtis
When You Reach Me by Rebecca Stead
    Missing May by Cynthia Rylant
Summer of the Swans by Betsy Byars
    Zach’s Lieby Roland Smith

They have worksheets with standard questions (characters, plot, etc.)

I think you mean The Sleeper and the Spindle. It's been on my TBR list since it was released; I love Neil Gaiman.

My nephew (rising 6th grader) read The Graveyard Book by Neil Gaiman for Summer Reading and loved it. By looking at it I think my rising 4th grader would enjoy it as would I!

    I've read that; it's fantastic!

However, I do recommend reading it before your 4th grader does before handing it over, due to some of the plot elements. (I'm sorry for the vagueness but I'm really trying not to be spoiler-y and finding it hard to balance the two.)

Another Gaiman novel that I can recommend wholeheartedly (and with no preview) for your 4th grader is Fortunately, the Milk, which takes the basic premise of a "shaggy dog story" and runs with it. In the US edition, the illustrations are by Skottie Young; in the UK edition, they're by Chris Riddell. No matter which version you get, you'll get a great story.

DD1 entering 7th, So Far From the Bamboo Grove by Yoko Kawashima Watkins (who comes to the school each year to speak to the 7th current grade class).

DD2 entering 6th, Tuck Everlasting by Natalie Babbit.

Fish in a Tree is really good, I read it when DD1 read it, I think she was in 5th.
