Anyone have a Nanny/Au Pair/Family Daycare contract they can share?

I am preparing to return to work and my mother will be caring for my 8 week old during the day.

We live in a multi generational household so the baby won't need to "leave" the home.

I was hoping to find some sample contracts to see how other families have addressed similar childcare scenarios....i.e. do you include a prefered schedule in the contract, what the care taker will and won't be expected to do or is it limited to fee information.
