I'd recommend buying a new mattress just due to the fact that your bassinet has gotten so much use through the years. I bought a replacement mattress for my bassinet last year from Babies R Us and it was just a standard oval shaped foam pad covered with vinyl. It wasn't too pricey, I think under $30. Sorry, don't know much about online sources, but I did a quick google and shopping.com search and they seem to be pretty readily available.

We've been using a hand-me-down bassinet. It's been passed around the family over the last 5 years or so, including DS, and DD is now the 4th child to sleep in it. Whenever I lift up DD, I notice an indentation where her head was. I'm thinking I should replace the mattress. Does it matter what kind I get? Where can I purchase just the Bassinet mattress? Is there a good online source? Or, do all of the bassinet mattresses do this?
