If you have access to a laser printer, you can make them yourself. Just use Avery weatherproof labels 5520. I have made several for DD and friends. They have held up throught the dishwasher and are much cheaper than the commercially available products.

I bought iron on and sticker name labels from an ebay seller loads_of_labels . They are from the UK, but even with the overseas shipping, the price was really good. I have been happy with the quality and they are still stuck on the sippy cups I put them on last year. I don't use my dishwasher though, so I can't speak for how well it would hold up in there. The iron-on labels have held up well too.
I was going to order them from Stuck on You, but they have a strict policy about only one name per label. I wanted one sticker with both DDs' names on it, so they wouldn't let me order it like that even though it fit in the letter limit. The ones I have are not as cute, but for the price and customization I wanted, it was fine with me :).

Another vote for Mabel's Labels. They are cute and last forever. Ours have been through the dishwasher literally hundreds of times.

I love the bumpy name labels from inchbug, but they are for bottles, sippy cups, and jars, not toys and things.
