View Full Version : Medical authorization letter??

My husband and I are leaving for a cruise next week and my mom is taking care of my DC. I was going to write a letter granting her authorization to make any medical decisions for my DC while we are gone.

Done anyone know if a Dr./hospital will accept this? Also what should it say? Do both my DH and I need to sign it?

Nothing like waiting until the last minute?!

yes they should accept it though they may try to get phone permission from you to cover their *sses in case of lawsuit, even with your authorization form. i would look around for a legal doc's website that you can download and personalize one for a price. you should both sign it and you will probably need it to be notarized. call your ped and see if they happen to have one. good luck! and have fun on your cruise!

If you still have time, call your peds office. Rules can vary by state. Here's the one my doctor's office (in Michigan) has on their website, for example:

I followed one of the examples and called my Dr's office. They said they and most any hospital would accept it!
