ISO fill-in-the-blank thank you notes

Any suggestions for where to find fill in the blank thank you notes? The kind where you just fill in the name of the person and the name of the gift.

With a new baby I'm burnt out on writing thank you notes, and would like to take the easy route for DS's upcoming birthday.

I have seen these at Target in the thank you note section. They only have a few choices. I have also seen them at Papyrus and Hallmark. I did a google search and found this website. I have never used them, but they look like they have a lot of choices:

Yep, Target! We have used these a few times for Joshua's gifts. They have had baby gift thank-you notes, birthdays, Christmas, etc. Look in the isle with all the note-cards.
