How do I measure rise?

Title says it all...and should he be wearing anything while I measure?

I made the mistake of measuring rise without using anything and quickly learned that my measurements were very off. One of the WAHMs told me to have Nate wear a trim diaper (I used a disposable) and measure from the front of his waist, through his crotch area and up to his waist in the back. This is rise.
