of the Sunday paper. There was not a form.

It just listed the website to visit to obtain the
form. On the website, there is a link to print
out the form.

Just got my book- it great. I thought it would be a little softcover pamphlet thing. It's a beautiful hardcover that must cost $25-30.
Now I'm wondering, why would they give them out for free? Are they going to flood me with junk mail or something? ;)
