Scholastic Parents and Child has one, Fit Pregnancy has one and Parents magazine has one.

Snagged one from my ped. office yesterday. We were in the room as the nurse was talking with us and I spotted on the back of the door. As soon as she left, I grabbed it and it was still in there. DH was in awe that I now know where to find the coupons by memory!
