toddler bed to straight to twin bed?

DD is only 18 mo so this is really in advance as we intend to keep her in her crib as long as possible. But I'm wondering how you transitioned DC from crib to bed. Crib/twin mattress on the floor? Straight to regular bed? Did you use side rails?

I did a twin mattress on the floor, and used a pool noodle under the fitted sheet. Worked perfectly!

Both mine went straight to twins w/ bedrail.

DD fell out of bed a couple times w/ no rail so it's been there ever since. I was thinking about taking it down last fall(at 4.5) and a friend of hers fell out(after her rail was taken off)and broke her clavicle. We had a major bone-breaking incident a year ago so we're not even considering removing our kids' rails for the foreseeable future.
