Can I pump milk from each breast into one (the same) bottle/storage bag, or do I need to use a separate bottle/bag for each breast? I have checked my breastfeeding books, and cannot find an answer!

I would put it all together. It's all breastmilk- I don't think the side matters. :)
I have put it all together, btw, and it has been fine.

It only matters if you have a foremilk imbalance or if you pump a full pumping on one side and just foremilk on the other etc.
I pump both sides in the mornings. I pump one side fully and it fills one bottle plus an ounce + in a second bottle. My second side never pumps a full bottle, so I pump in to the same bottle that has the one ounce in it already - but that one ounce is a fat rich hindmilk. So, I take the two bottles at the end and mix them together and then measure out portions for storage (I store some in 4 1/2 oz increments and some in 1 and 2 oz increments.
