View Full Version : Does your ped charge $ for school forms?

I took in one form for the ped to fill out today, which is required by DD's preschool (public school), and they told me that there will be a $5 charge. What?! I asked why would I pay $5 for someone to check a couple of boxes and sign their name. The receptionist told me to call the office manager.

So, does your ped charge to fill out forms? Is this standard practice?

If they are not done at the time of the physical yes they charge $5

Yeah, if it's not their appt., they do, I thought I had bypassed it this yr but her pre-school sent me another health form, even though I had gotten one for camp, and I will be taking it to her sister's appt, I don't even need the dr signature, just someone, even a nurse, but I think they'll charge me the $5.

I think mine is $10-- it seems a lot, but I understand that it takes administrative time (I guess).

No - at least they didn't. I took DS1's in when I had a WBV for DS3 and they filled it out no charge.

Okay, well, I feel better now. I was totally annoyed when she told me about the $5. I guess I'll just have to suck it up and pay.

DS2 had an appointment for vax today. I brought DD's form and they filled it out after the appointment while we waited. They made a copy for their records and that was it. No charges, no problems. Her physical was in June.

No, but I've always brought in forms to be completed during a physical or check-up of some kind.

However, I can't tell you how many times I"ve asked for copies of their immunization chart and they always make the copies or fax them for me immediately with no problems.

I've always gotten them after her checkup with no fee. They do charge $10 to replace the original if you happen to loose it though, which I guess seems fair since I got one for free (they tell us to make copies).

No, they never have. Never to mail them to potential preschools or elementary schools (when we were applying) or to fax them. I'm sure they make up for it by charging more for the visits, though. :wink2:

No fees, they automatically give you a copy when the kids have their annual physical, but somehow they get misplaced by the time Summer camp forms are due so I ALWAYS end up requesting more copies, there has never been a charge.

Mine does not charge for forms at any time. I have been investigation new practices since we moved and will not go to a practice that charges, just because I think it's rude.

You're all lucky. Ours charges $15 per form. It used to be free if done at a well check, but they changed about a year ago "due to volume".

Our ped. gives us a form at the annual physical which is free. If you lose it and need another copy then they will charge you.

Ours changed in mid-2006 to free to $10 to complete forms if NOT at annual physical time. There are standard forms from the state, so I just take them with me each year. I just did for my 2 year olds well child appointment in June although right now I don't think he'll be going to any KDO or pre-school program in the next year, but easier to have them than make the trips and pay the fees later.

Virginia Pediatrics? Although I think most peds around here have a similar policy.

Wow, I have to say that I'm a little surprised that this is such a common policy. Our ped doesn't charge.

I think it depends on where you live. Here there are a thousand different day camps and preschools, PLUS the ones needed for K/1st entry and they ALL require this form. The peds here are really swamped. I wonder if people were taking in stacks "just in case" and so this policy was the result.

My favs are the "old" guys, Baugh, Libby and Weinstein. Fishelberg is good too. I like the PA Crowther too. I was very sad when Puppo left (that may have been before your time though).

Baugh is the one who went to bat for us with the Fairfax NICU and got Amy discharged when they didn't want to because she wouldn't take a bottle. Little did they know that she would NEVER take a bottle. :ROTFLMAO:

Our ped fills out 2 pages that are standard in the office: An overview page (ht, wt, allergies, etc) and a vaccination page with dates of all vaccines given. This is free at the time of the annual office visit. I attach these to the forms the schools give out and write "See attached." If I were to insist that the dr fill out the school's form, then they would charge me. Seems fair.

Beth - this was my practice several years ago when I lived closer by in Fairfax City. I loved Baugh, too, and also Libby. And there was a woman there I liked but I can't remember her name now.

Mine just started charging $5 about a year ago. I understand how sometimes it's legit, but the last form they literally just filled out her height, weight and age which I could have done myself

Filling out forms is free if I present them at any kind of doctor's appointment, including a sibling's visit (so I can bring DD1's forms to an appointment for DD2). Otherwise, there is a $15 fee.

Filling out forms and copies of immunizations, etc. are always free for us. I would be mad if I had to pay for them. Based on all the responses, I guess I have been spoiled.

Our ped doesn't charge for this, but our form requires a doctor's signature and some other info. They are basically charging for the doc's time to fill out the form. Once I started looking at it that way, it stopped bothering me.

We've never been charged for anything like that, regardless of when it is done.
