Does he have any Imaginext sets? My DS is 3 and DD is almost 5 and they play with them all the time. Gorilla Mountain has been a hit for a while, but the new favorite is the Batman Cave and accessories. My almost 6 year old nephew was here last week and said he wants one for his birthday. They have a new one that looks fun, called Dragon World Fortress too.
Does he like building? Magna tiles are great, and so are Exobones Deluxe. It's a bug building set that my kids have a lot of fun with. There's also one called Globonz that's a glow in the dark dinosaur building set.
Leapster?
TAG set?
Golf clubs? (Costco has cute kid sets online)

DS1 was and is still very much into Legos. We are getting quite a collection of Lego City sets. Games were also big hits. Ravensburger's Rivers, Roads, and Rails is a good one if he's into those things. (Beware, though, that it takes a long time to finish the game. I think it took us almost an hour last time we played.) Blokus is great too as games from Blue Orange and Gamewright.

My ds was all about the $$ Lego sets. He thinks it is beyond awesome to get an $80 Lego set (believe me, that is the only present he gets if we get that!).

Bike?

Board game- Amazing Labyrinth

DS just turned 5 and Legos have been the best.gift.ever.
He plays with them for hours....makes up stories, plans, buildings, etc. It is so great to watch!

For some other ideas...
Marble run/building sets
Bike
Dress up clothes (we got an awesome firefighter one from the grandparents)
48+ piece puzzles

Have fun!

Another 2nd on Magna Tiles. Both my kids have been really creative with them and we've ended up buying more so we can build more. DS also enjoys dressing up as a Pirate so we have Pirate hats, etc.

My two older boys are NUTSO about star wars legos. I notice that a lot of boys that are 4-6 yrs old are REALLY into star wars, so I don't think you can go wrong getting them a star wars lego set.

DS is 5.5.

He likes anything with:
-Lego (we don't buy many as MIL saved DH's collection thankfully!)
-Playmobil
-Anything vehicle related, especially playmobil and vehicles combined (he likes the car repair and tuning shop for example, and the camper was his first PM toy and he still likes it). RC cars, matchbox cars...pretty much any and all vehicles. He'd like a Bruder truck for sure.
-science-y stuff like snap circuits. He's into robots right now and we just picked up the snap circuits RC rover and he's having a lot of fun building and playing w/ that. Any kind of science kit, etc. is something he likes.
-enjoys playing magnatiles w/ his sister still
-marble maze is still enjoyed
-books. Fiction and non fiction. He likes the Eye Wonder series by DK books a lot.

Those things consume most of his time.

Besides the many things listed here that T loved (esp. science kits, legos, books) he also loved and continues to play with:

T-ball set

and SuperStructs Wacky Machines. Many, many hours of building fun. He loves it that the stuff "works". It was his one present from us when he turned 5 and he was in 7th heaven for weeks and weeks and it still gets used.

DS loves his Matchbox Mega Rigs. Looks like this year they have a Pirate Ship. http://www.toysrus.com/product/index.jsp?productId=3504333 Last year's Shuttle is awesome if you can find it for a decent price (should be $45 or less). All the sets are compatable and DS loves building different "machines". You can find some of the older sets on ebay- shark ship, sub, fire brigade, etc.
