In general, she said, parents should alert their child's doctor if:

An infant is under 3 months old and has a fever of 100.4 F or above.
An infant between 3 and 6 months old has a fever of 101.5 F or more.
An older child has a fever of 103 degrees Fahrenheit or higher.
Any child has a fever accompanied by lethargy, headache, skin rash, trouble breathing or dehydration.

My husband has a huge fever phobia. He freaks out at the slightest temps and asks if I should be giving them cool baths or something. When I did my out-patient clinic rotation in nursing school, I took the time to photocopy the entire fever section from the telephone triage book just so he could refer to it. Not that he has but still...

I'm more of the "treat the discomfort" type. If they are hot and comfortable, I don't worry about it but if they have a low grade fever and are uncomfortable, I give them something for it.
