Ikea makes great, flat ones in their baby section. They're hard to find b/c they're rolled up. My mom made a fleece case for it. I've also found travel pillows at Target.

I got a couple from Mr. Bobbles Blankets when I bought him some toddler blankets. I love that I can toss them into the washer/dryer when I need to.
We got ours at IKEA. They sell a toddler pillow and comforter set, and really cute pillow cases and comforter covers in the Children's IKEA section.

Just came back from Walgreen's where I got mine for $3.99, too, I think. It came with a white pillow case (though it's thin, I'm assuming), but you can also buy additional cases for about $3 for a pack of two (or two packs, 4 total, for $5). It was in a random spot, on the bottom shelf by the school supplies (tagboard, etc.). Also had some fleece blankets and other blankets by it.
