In general, we cancel our nanny when DS is sick. When I was still nursing, I'd stay home in order to get him extra fluids. Now, DH usually stays with him because we figure there's no point exposing her to the germs (same if one of us is home sick). But, our nanny is only part-time and DH has a fair amount of flexibility in his schedule, so we can do that. And if DH had a commitment he couldn't miss, we wouldn't worry about our nanny taking care of DS with a cold or something relatively minor.

If our daughter is sick with a mild fever or cold she stays home from preschool and either our nanny (or a temp agency nanny if our nanny isn't available) stays with her. Both of us work in the medical field so (ironically) it's really difficult for us to call in sick. But if she's very sick one of us would go home (like the time she had flu/high fevers from probable H1N1, DH went home to take care of her--I was on call and couldn't leave work).
