You could use it bare, but the fabric is kind of crappy ... and if your LO pees through their clothing while they sleep, then you've got a urine-soaked "mattress" and no good way to clean it.

There are washable pads that you could use as a waterproof layer under the sheet; I have seen them near the changing pads and regular bassinet sheets at my local Target.

That said, I don't use any sheets or pads in my PNP, but my LO doesn't sleep there!
