Just the pad has been fine. We have it tacked to the back of the dresser.
Although that is soon to end because The Biscuit is now longer than the changing pad (and super wiggly)!

We just plopped a contoured changing pad on top of the dresser. Works great!

Another vote for strapping a changing pad to the dresser followed by a waterproof pad or towel on the floor whenever baby becomes mobile. I was doing all floor changes by 6 months with both of my little acrobats.
This was the pad I used: Serta Organic Contour Pad. That said, I think I changed DD on top of the dresser maybe 3 times ever. Our Pack n Play had a changing area, so that's what we used when she was an infant. Once we moved her to her crib, it was just easier for me to change her on the floor. I have a waterproof FuzziBunz mattress pad on the floor, so that's what I use as a layer of protection.
When she was an infant and EBF, I liked to change her on the Skip Hop Pronto changing pad (which fit nicely on the PnP changing area and then the contoured changing pad on the dresser). It's easily wipeable, so I didn't have to worry about getting the changing pad cover messy and having to do laundry!
ETA: We did secure the changing pad to the dresser with the strap and supplied screw. It was simple to do.

We also did the Cooshee pad right on top of the dresser. It's a shame it was discontinued, I thought it was a great item.
We have this changing pad, which comes with hardware to add snaps to the back of a regular dresser. Works beautifully.
