Is that a seatbelt OVER the harness?
My next question is, is the seat LATCHED, or is the seatbelt used to secure the seat AND the child?
If the seat is properly installed with LATCH, I think I would mention that the seatbelt does not need to be used over the harness. If the latter, well, I'm going to let someone else chime in on that.

Is the harness on the outside of the legs instead of between them? Oh my.
Yeah, I'd just say, "Hey, cute kiddo! I was just wondering if you also have the LATCH attached? If so, it's not necessary to use the seatbelt, too, might make it a little easier to get kiddo buckled up, too!"
Something like that.
Sigh, look at that baby chub, you just wanna rearface her (not in that seat, it's a cosco 'high back booster', an inexpensive but incredibly short FF only seat that comes in THE most adorable colors at Walmart).
If they don't take your advice seriously, I don't think it's the worst thing, hopefully it's LATCHed, though.
Back when seats when to only 40 pounds and sometimes two yo's hit 40 pounds, we used to debate strenuously whether it was a good idea to harness them in place then buckle them...there was some talk of the seat and child rotating wholesale out of the belt, but I never saw a crash test or heard of injuries, and the carseat reps at the conferences just couldn't even understand why anyone would do it, I don't think they believed me that people DID do it...
