I stick it on a bag of Disney or Star Wars mini-figs, or on a pack of Pokemon cards and put it all in a little gift bag. But a gift card in a card is totally fine IMO.

We do cash and Target cards too!
If there's time, we do a little something to open too - Kanoodle, a card game or a Squishmallow or Beanie Boo (I'm wondering if my kids will ever outgrow stuffed animals, but their friends seem to love them too).-Christine

We most often do $20 in cash now too. Sometimes just in a card and sometimes wrapped with a candy bar.
